History of scurvy, why does it matter today?

Some symptoms of scurvy. The disease was common among sailors and solidiers for hundreds of years, due to a lack of fresh fruits and vegetables in their diet.

Those who do not learn history are doomed to repeat it.

The history of scurvy is a stark warning, for those who insist in a pathogen cause for disease in the face of contradictory evidence. Scurvy is a terrible disease. The body, deficient in vitamin C is no longer able to make proper connective tissues. Late symptoms of scurvy include:

  • Bleeding gums that swell and turn purple
  • Teeth become loose or fall out
  • Skin easily bruises and bleeds
  • The opening up of previously healed wounds and scars
  • Bleeding into joints and muscles
  • Death

For hundreds of years scurvy was especially common among sailors, who spent month away at sea eating no fresh fruits or vegetables. Robbed of the vitamin C found in such foods, their bodies where literally unable to form the connective tissue that holds us together.

The cause and cure was discovered over and over again. Each time this information that would have easily saved thousands from a terrible death was discarded because it was not what people wished to believe.

Sir Richard Hawkins

In 1593 the crew of British ship led by Sir Richard Hawkins discovered that eating oranges and lemons in Brazil cured the condition. Regardless, Hawkins stayed with the current wisdom of the time in believing the scurvy was caused by unsanitary conditions on the ship. Thus what could have been the discovery that ended scurvy once and fall became a footnote in history.

Francois Pyrard

The Voyage of François Pyrard of Laval to the East Indies, the Maldives, the Moluccas and Brazil

About the same time French negotiator Francois Pyrard wrote a detailed account of long voyage to South East Asia. Aside from being held 5 years captives by locals, his crew also suffered scurvy. According to his account the severe nutritional deficiency was "very contagious even by approaching or breathing another's breath." His crew did experience reversal of scurvy by eating citrus fruit. This discovery was once again discarded because everyone knew it was caused by unclean conditions on ships.

James Lind publishes the cure for scurvy

In 1734 the captain of a British ship marooned a sailor with a particularly bad case of scurvy on an island. As everyone knew, scurvy was contagious and the captain didn't want other members of the crew to catch it. Eating food available on the island the sailor recovered. He later found passage back to England on another ship.

This prompted navy surgeon James Lind to investigate a possible cure for scurvy. Lind concluded that some nutritional factor found in citrus fruits, lacking from the diet over long journey, was the cause of scurvy. He published this in a book in 1753. This information was ignored.

James Cook

In 1769 the English captain James Cook discovered, one again independently, that fresh vegetable and citrus fruits worked as a treatment for scurvy. Regardless, he insisted that good hygiene and fresh air was essential as well. The significance of his discovery was thus missed, because he did not leave the contagious disease paradigm.

Limey

In 1795 the British navy began adding lime or lemon juice into sailors diets. This is the origin of the slang work for sailors, Limey. At this point the debate over the cause and treatment of scury should have ended. For hundreds of years it was treated as a contagious disease. This prevented nothing. In multiple instances the disease was prevented and treated with fresh fruits. Yet this was not enough. At bacteria were discovered, and with that the germ theory of disease, once again scurvy was called a contagious disease.

Jean-Antoine Villemin

Jean-Antoinie Villemin was a member of the Paris Academy of medicine. His great discovery was that tuberculosis is an infectious disease. This led him to become a passionate germ hunter and thus he turned his work to scurvy. A paraphrased excerpt from his writing:

Scurvy is a contagious miasm, comparable to typhus, which occurs in epidemic form when people are closely congregated in large groups as in prisons, naval vessels and siege's... We have many examples of well-fed sailor and soldiers going down with scurvy, while others less well fed do not. Also, we have positive evidence of the spread of the disease by contagion - for example, the introduction of scurvy in to French military hospitals by veterans returning from the Crimea, and the rapid spread of scurvy from one sailor to another in naval vessels.

A key point is that just because there is a disease clusters, that does not mean it must be contagious. All there needs to be is a common factor among people in the cluster causing the disease. In this case, it was a diet absent of fresh fruits and vegetable's.

Odd are you probably never heard about ptoamaines. It was theorized that bacteria poisoned meat, producing the "ptomaines," which then caused scurvy. In 1899 the Royakl Society of London funded research in which monkeys where fed rotten meat. When the monkeys got this, that only inspired the bacteria hunters to search for the bacterial cause of scurvy.

The search for scurvy causing bacteria continues

The germ theory of disease and discovery of actual germs that could be proved to cause illness created a craze of bacteria hunters. Experiments continued, searching for the scurvy bacteria in patients. Transplanting bacteria found in one animal with scurvy into another. This research led to no scurvy breakthrough, but it did have real world consequences. In WW1 doctors blamed scurvy in soldiers on bacteria and unsanitary conditions. Soliders at Gallipoli in 1915 developed scurvy from poor food rations.

During WW1 soldiers would get scurvy due to poor rations. It was still treated as an infectious disease.

Finally, vitamin C is purified

It was not until the purification of vitamin C and research into its function that the bacteria hunters search for the scurvy pathogen ended. The reason why the search for the cause of scurvy took so long was summarized chemistry professor of the University of Edinburgh, C.P. Stewart in 1953:

One factor which undoubtedly held up the development of the concept of deficiency diseases was the discovery of bacteria in the nineteenth century and the consequent preoccupation of scientists and doctors with positive infective agents in disease. So strong was the impetus provided by bacteriology that many diseases which we now know to be due to nutritional or endocrine deficiencies where, as late as 1910, thought to be "toxemias." In default of any evidence of an active infecting microorganism they were ascribed to the remote effects of imaginary toxins elaborated by bacteria.

Over 300 years until acceptance

It took over 300 years from Richard Hawkins 1593 discovery, until the 1930's for medicine to accept that scurvy is caused by a nutritional deficiency. Although scientists may not have yet had the technology to isolate vitamin C, the empirical evidence that a diet lacking in fresh fruits or vegetables caused scurvy was obvious. It was the belief that some infectious agent had to be the cause, which kept scurvy going long after it could have been eliminated.

All this would be nothing more than historical footnote, except similar processes continues to go on today. At first scurvy was a "miasma." As science evolved it became a bacterial infection. Then it became toxemia caused by bacteria. If the viral theory of disease, along with virus hunters existed back then, they undoubtedly would have been chasing it down. Searching scurvy patients for any incidental virus found to blame it on.

This article is mostly a summary from Peter Duesberg's book, Inventing The Aids Virus. Published in 1996. This book is if anything, even more relevant today than when first published, in understanding the history of virology and how that field functions.

Clinoquiniol is a topical antifungal. It used to be sold for internal use, with disastrous results.

The following is a summary from the book Inventing The Aids Virus by Peter Duesberg. Although published in 1996, the book is still relevant today. I am sharing this in the hope it will encourage people to question declarations handed down to them from scientists and media. It is also important to understand that scientists are people just like everyone else. They have their biases and may pursue research with the tunnel vision of their narrow range of expertise.

A new epidemic emerges

In 1959 patients in Japan began presenting with what appeared to be a new viral infection. Symptoms included nerve degeneration, paralysis of limbs, along with intestinal problems which manifested as internal bleeding and diarrhea. Outbreaks happened locally in clusters, including even families. Clusters centered around hospitals, peaked in the summer and there were higher rates of SMON among medical workers.

Considering the similarity of symptoms to polio, researchers hunted for the viral cause of SMON. This continued for years. It was a mysterious virus as patients did not have typical signs of infection such as fever or rashes. Blood tests showed no infection. Meanwhile, SMON carried on, even causing blindness in some.

In 1964 the disease was given the name "Subacture Myelo-optico-neuropathy" or SMON. The Japanese government funded a commission to investigate SMON. This included several virologists. Soon after there was a major breakthrough. An echo virus was found in excretions from SMON patients. Echo viruses had already been discovered in polio research. They where known to infect the intestines, but where not thought to cause disease. Now an echo virus was being found in the digestive system of SMON patients.

In 1965 research was published that the virus had been linked to SMON thus demonstrating the disease's cause. This was followed by more years of research by other scientists trying to confirm the findings. They were not able to do this. The commission ended its research in in 1967, failing to find a cause. Then in 1968 researchers found a Coxsackie virus in patients. This turned out to be a laboratory contamination.

More money and research for the virus hunters

In 1969 the Japanese government funded a new commission. This one with 10 times more funding than the first one. This commision headed by virologist Reisaku Kono would also be largely a hunt for the viral cause of SMON. For three years research was done into finding a "slow virus". This was based off of the work of virologist Careleton Gajdusek who claimed to have found such a virus as the cause of kuru. The "slow virus" cause of kuru was not based upon isolating a virus. Instead he injected unpurified mixtures from diseased brains into monkeys, and then showed replication of neurological symptoms. So now, similar research was being done in Japan to find the viral cause of SMON.

In 1970 researcher Shigeyuki Inoue claimed to have found a the viral cause of SMON. Although his results had not been replicated the media published his results. This lead to hysteria. Patient families would refuse to see them from fear of contracting the disease. There were even suicides among patients based on this work.

Clinoquinol

Thankfully, not all SMON researchers where virus hunters. In July 1970 it was found that almost all SMON patients in a hospital had used the drug clioquinol. Drug toxicity also matched many feartures of SMON. The drug was prescribed more in the summer, it was not prescribed to children in high doses (SMON did not appear in children) and the lack of typical viral infection symptoms. The clusters of cases, which was assumed to be caused by a contagious disease, simply reflected the pattern of clinoquinol use. In September 1970 the Japanese government banned the sale of clinoquinol. The disease epidemic was soon over. Nonetheless, it would take a few more years for the viral hypothesis to be abandoned by all the virus hunters.

Later on the pharmaceutical company which many clinoquinol was successively sued. They had ignored reports from all over the world of symptoms appearing wherever clinoquinol was used.

So why did it take so long for the non-viral cause of SMON to be found? Two reasons. Clinoquinol was commonly used in Japan to treat the gastrointestinal symptoms of SMON. Doctors did not wish to believe that their treatment was actually the cause of the disease. The second reason was the Japanese government funding primarily virologists. They would then go on the hunt for the viral cause of disease. This all happened after polio, and so researchers were looking for a similar cause. Preliminary data that clinoquinol was the cause of SMON was found early in the investigation. It took years for this to be pursued.

What causes high blood sugar?

Most of the time we know what causes high blood sugar. Eating too much food. Especially simple carbohydrates and junk food. But there are other factors which can elevated blood sugar independent of diet.

Other causes of high blood sugar include:

  • Stress and cortisol
  • Nutritional deficiencies

The adrenal glands, cortisol and the stress response

The adrenals are two small glands which sit on top of the kidneys. Their job is to make numerous hormones which regulate stress response including cortisol, epinephrine and aldosterone. For the purpose of keeping things simple, I’m just going to focus on cortisol.

Cortisol is out "Fight or Flight" hormone. Normally it follows a diurnal, rising to it’s highest point in the morning, and then slowly decreasing during the day. Ideally it is lowest at night when we go to sleep.

Chronic stress can deregulate this pattern. Stress is simply the body’s adaptation to any extra demand. This can be psychological stress, or physical stress from an injury. The body's physiological response is the same. The adrenals release cortisol. It’s job is to get more blood and nutrients to the heart, lungs and brain to help you survive an emergency.

Short term elevations of cortisol are essential for survival. But when stress in our daily lives does not go away, cortisol levels get thrown off.

Cortisol may be high, or in some cases the adrenals became exhausted and cortisol is too low. Neither is good. It is also possible for cortisol to be normal for some parts of the day, but deregulated at other times.

Lab tests for blood sugar

Since cortisol raises blood sugar, that means even if you eat a good diet, stress by itself may cause a blood sugar issue. Blood test can help figure this out.

  • Blood glucose: This is your blood sugar levels at any one moment. This test helps diabetics regulate their medications, supplements or diet. A single reading should never be used to determine if someone is a diabetic or has a blood sugar problem. It's possible for blood sugar to be ok during part of the day, and very much off at other times.
  • HgA1c: Used to test blood sugar over time. Hemoglobin is in our red blood cells. Its job is to bind oxygen. Over time blood sugar will glycosylated hemoglobin. The higher the blood sugar over time, the higher HgA1c will be. This test can not be used for short term rises in blood sugar. Instead if shows if blood sugar has been elevated over several months.

Mysteriously high blood sugar and cortisol

If you follow the following pattern, you may be experiencing a spike in cortisol while you sleep, causing high blood sugar:

  • Elevated HgA1c
  • Normal blood glucose levels on blood tests
  • You are not eating a high carbohydrate, or high sugar diet
  • You wake up in the middle of the night for no reason (not all the time, but common)

How to test for middle of the night spikes in blood sugar and cortisol

Go to any drug store and buy a glucometer. I recommend getting a cheap one, as you probably are not going to be using it that often. Take your blood sugar at night and in the morning. If you wake up in the middle of the night, take your blood sugar then as well. It's normal to see some increase in blood sugar as your sleep. This is part of your body getting ready to wake up. However a very large spike can indicate that cortisol is too high. In some people cortisol is normal during most of the day, but goes up way to high at night.

The other test is a saliva cortisol. Theses are commonly used by naturopathic doctors, or other functional medicine practitioners. One of the advantages in using a saliva cortisol tests is that samples can be taken multiple times during the day and even in the middle of the night. If I have a client who is waking up in the middle of the night, I have them take a cortisol sample then, so we can see exactly what is happening.

If blood sugar is spiking in the middle of the night, there is a good chance it is cortisol. The glucometer blood sugar test is therefore a way to infer if cortisol is going up to much at night. The saliva cortisol is a direct measurement.

The advantage to testing blood sugar is you get immediate results. Taking a saliva sample, sending it off to the lab and waiting for results takes about two weeks. You also need to either order a kit from a company that sells it, or get one from a practitioner you are working with. Neither test is that difficult to do, so doing both is really the best option for difficult cases.

How to fix elevated cortisol

The real answer is going to be different for everyone. When working with clients I often use herbs or homeopathics which are best individualized. There are also many different way to lower stress response with lifestyle modifications such as meditation and exercise. Each person needs to find what works for them. In the office I also use a system called Neuro Emotional Technique to help the body release emotional triggers it is holding onto.

Some common supplements for stress, cortisol and blood sugar

These are some supplements that often work. Since everyone is different I can't guanantee these will work for everyone. But, without knowing anything more about a case, other than there is a stress response at night, these are the supplements that come to mind to try first.

  • Seriphos or phosphatidyle serine. Both of these are similiar. Seriphos will actually get converted into phosphatidyle serine in the body. For some people one may work better than the other. Phosphaidyle serine blunts the adrenals from being signaled into making too much cortisol. Doses can vary a lot. My last year in naturopathic school I developed middle of the nigh insomnia. I woke up about 3AM every night, for 8 months in a row. I didn't discover the cause until finally getting a cortisol test from my schools clinic. My cortisol was more than double what it should have been in the morning. One of my teachers told me to try 200 mg of phosphatidyle serine. It did nothing until I took 800 mg before bed.
  • Chromium: this is the most commonly used supplement for elevated blood sugar. Without chromium insulin can not function properly. It's very simply, but does work.
  • Ashwagandah: A very common herb for the adrenals. Some herbs which are used for the adrenals such as rhodiola, American ginseng or licorice can be very stimulating. Ashwagandah is more gentle and over time helps to repair the nervous system. Not a quick fix but generally very good. There are other options, but I can only recommended the bests helps I know if going over a case one one one.

As a side notes, some people may wake up in the middle of the night because cortisol and blood sugar are too low. In those cases eating a snack at night, or taking the herb licorice may help. The herb valarian is also generally for good for sleep. For those those with too much heat (Pitta in Ayurvedic terms) it can actually make their sleep worse. This is why going over a case properly and treating the person is much better than trying various supplements based only on their symptoms or disease.
